CONSENT TO LEAVE FOR LUNCH

Morning and Afternoon Tea will be provided, however lunch is not. Students are welcome to bring lunch, or with parental consent they may visit one of the local Crows Nest Cafes/Take away Restaurants. Please advise if you are happy for your child to leave the building during the lunch period.

**Students under 12 years of age will be accompanied by a coach or BIG MUSIC staff member at all times unless you give permission for them to do otherwise.

PERMISSION TO LEAVE UNACCOMPANIED

Do you give consent for your child to leave BIG MUSIC unaccompanied after the completion of Holiday Rock Camp each day.

PERMISSION TO LEAVE - Other Parents/FamilyDo you give consent for your child to leave BIG MUSIC with another student's family? If so, please include their name, contact details and what days they will be collecting your child.

MEDICAL CONSENT

Should BIG MUSIC at any time consider that my child requires medical, dental or hospital assistance and they are unable to contact me on the numbers listed above, I consent to you obtaining this assistance at my expense.

CONSENT for PHOTO/VIDEO

Do you give consent for Big Music Australia Pty Ltd to publish still photographs and film footage obtained during the course of activities and events associated with Big Music, that may contain images of your child?

These images may be used as part of any promotional material and images created and published by Big Music Australia Pty Ltd from time to time including, but not limited to, print media (e.g.; brochures, advertisements) and the Big Music website.

TERMS & CONDITIONS - Program Changes

At the coaches discretion students may be moved between programs or groups based on their ability. This will be in the interest of providing everyone with equal opportunities during the program. 

TERMS & CONDITIONS - Behaviour and Conduct

Big Music has the right to remove students from the course at the Coaches discretion should there be any inappropriate behaviour or conduct. This may be done without warning and a full refund will be provided. 

TERMS & CONDITIONS - Mobile Phones

Mobile phones are not permitted for use for the duration of the program. All phones will need to be left in the student's bag or with reception at the commencement of the day. If a student needs to contact a parent or carer, this can be done from reception.
